1.Ayat al-Kursi
From the Quranٱللَّهُ لَآ إِلَـٰهَ إِلَّا هُوَ ٱلْحَىُّ ٱلْقَيُّومُ ۚ لَا تَأْخُذُهُۥ سِنَةٌ وَلَا نَوْمٌ ۚ لَّهُۥ مَا فِى ٱلسَّمَـٰوَٰتِ وَمَا فِى ٱلْأَرْضِ ۗ مَن ذَا ٱلَّذِى يَشْفَعُ عِندَهُۥٓ إِلَّا بِإِذْنِهِۦ ۚ يَعْلَمُ مَا بَيْنَ أَيْدِيهِمْ وَمَا خَلْفَهُمْ ۖ وَلَا يُحِيطُونَ بِشَىْءٍ مِّنْ عِلْمِهِۦٓ إِلَّا بِمَا شَآءَ ۚ وَسِعَ كُرْسِيُّهُ ٱلسَّمَـٰوَٰتِ وَٱلْأَرْضَ ۖ وَلَا يَـُٔودُهُۥ حِفْظُهُمَا ۚ وَهُوَ ٱلْعَلِىُّ ٱلْعَظِيمُ
Allāhu lā ilāha illā huwal-Ḥayyul-Qayyūm, lā ta'khudhuhu sinatun wa lā nawm, lahu mā fis-samāwāti wa mā fil-arḍ, man dhal-ladhī yashfa`u `indahu illā bi'idhnih, ya`lamu mā bayna aydīhim wa mā khalfahum, wa lā yuḥīṭūna bishay'im-min `ilmihi illā bimā shā', wasi`a kursiyyuhus-samāwāti wal-arḍ, wa lā ya'ūduhu ḥifẓuhumā, wa huwal-`Aliyyul-`Aẓīm.
Allāh - there is no deity except Him, the Ever-Living, the Self-Sustaining. Neither drowsiness overtakes Him nor sleep. To Him belongs whatever is in the heavens and whatever is on the earth. Who is it that can intercede with Him except by His permission? He knows what is [presently] before them and what will be after them, and they encompass not a thing of His knowledge except for what He wills. Source: Quran 2:255Hisn al-Muslim no. 75 Reciting it morning and evening comes from the narration of Ubayy ibn Ka'b, graded sahih by al-Albani (Sahih al-Targhib 662) and with a good chain by al-Mundhiri. Al-Albani later judged the morning-and-evening wording an irregular (shadh) addition (al-Silsilah al-Sahihah 7/743). Reciting it before sleep is in Sahih al-Bukhari (2311).
2.Al-Ikhlas, al-Falaq and an-Nas
From the Quran قُلْ هُوَ ٱللَّهُ أَحَدٌ ٱللَّهُ ٱلصَّمَدُ لَمْ يَلِدْ وَلَمْ يُولَدْ وَلَمْ يَكُن لَّهُۥ كُفُوًا أَحَدٌۢ ۞ قُلْ أَعُوذُ بِرَبِّ ٱلْفَلَقِ مِن شَرِّ مَا خَلَقَ وَمِن شَرِّ غَاسِقٍ إِذَا وَقَبَ وَمِن شَرِّ ٱلنَّفَّـٰثَـٰتِ فِى ٱلْعُقَدِ وَمِن شَرِّ حَاسِدٍ إِذَا حَسَدَ ۞ قُلْ أَعُوذُ بِرَبِّ ٱلنَّاسِ مَلِكِ ٱلنَّاسِ إِلَـٰهِ ٱلنَّاسِ مِن شَرِّ ٱلْوَسْوَاسِ ٱلْخَنَّاسِ ٱلَّذِى يُوَسْوِسُ فِى صُدُورِ ٱلنَّاسِ مِنَ ٱلْجِنَّةِ وَٱلنَّاسِ
Qul huwallāhu aḥad. Allāhuṣ-ṣamad. Lam yalid wa lam yūlad. Wa lam yakun lahu kufuwan aḥad. · Qul a`ūdhu birabbil-falaq. Min sharri mā khalaq. Wa min sharri ghāsiqin idhā waqab. Wa min sharrin-naffāthāti fil-`uqad. Wa min sharri ḥāsidin idhā ḥasad. · Qul a`ūdhu birabbin-nās. Malikin-nās. Ilāhin-nās. Min sharril-waswāsil-khannās. Alladhī yuwaswisu fī ṣudūrin-nās. Minal-jinnati wan-nās.
Say, "He is Allāh, [who is] One, Allāh, the Eternal Refuge. He neither begets nor is born, Nor is there to Him any equivalent." · Say, "I seek refuge in the Lord of daybreak From the evil of that which He created And from the evil of darkness when it settles And from the evil of the blowers in knots And from the evil of an envier when he envies." · Say, "I seek refuge in the Lord of mankind, The Sovereign of mankind, The God of mankind, From the evil of the retreating whisperer - Who whispers [evil] into the breasts of mankind - From among the jinn and mankind". 3.We have entered the evening, and the dominion belongs to Allah
Authentic Prophetic duaأَمْسَيْنَا وَأَمْسَى الْمُلْكُ لِلَّهِ، وَالْحَمْدُ لِلَّهِ، لاَ إِلَهَ إلاَّ اللَّهُ وَحْدَهُ لاَ شَرِيكَ لَهُ، لَهُ الْمُلْكُ وَلَهُ الْحَمْدُ وَهُوَ عَلَى كُلِّ شَيْءٍ قَدِيرٌ، رَبِّ أَسْأَلُكَ خَيْرَ مَا فِي هَذِهِ اللَّيْلَةِ وَخَيْرَ مَا بَعْدَهَا، وَأَعُوذُ بِكَ مِنْ شَرِّ مَا فِي هَذِهِ اللَّيْلَةِ وَشَرِّ مَا بَعْدَهَا، رَبِّ أَعُوذُ بِكَ مِنَ الْكَسَلِ وَسُوءِ الْكِبَرِ، رَبِّ أَعُوذُ بِكَ مِنْ عَذَابٍ فِي النَّارِ وَعَذَابٍ فِي الْقَبْرِ
Amsaynā wa amsal-mulku lillāh, wal-ḥamdu lillāh, lā ilāha illallāhu waḥdahu lā sharīka lah, lahul-mulku wa lahul-ḥamd, wa huwa `alā kulli shay'in qadīr. Rabbi as'aluka khayra mā fī hādhihil-laylati wa khayra mā ba`dahā, wa a`ūdhu bika min sharri mā fī hādhihil-laylati wa sharri mā ba`dahā. Rabbi a`ūdhu bika minal-kasali wa sū'il-kibar. Rabbi a`ūdhu bika min `adhābin fin-nāri wa `adhābin fil-qabr.
We have entered the evening and with it all dominion is Allah's. Praise is to Allah. None has the right to be worshipped but Allah alone, Who has no partner. To Allah belongs the dominion, and to Him is the praise, and He is Able to do all things. My Lord, I ask You for the goodness of this night and of what comes after it, and I seek refuge in You from the evil of this night and of what comes after it. My Lord, I seek refuge in You from laziness and helpless old age. My Lord, I seek refuge in You from the punishment of the Fire and from the punishment of the grave. 4.O Allah, by You we enter the evening
Authentic Prophetic duaاللَّهُمَّ بِكَ أَمْسَيْنَا، وَبِكَ أَصْبَحْنَا، وَبِكَ نَحْيَا، وَبِكَ نَمُوتُ، وَإِلَيْكَ الْمَصِيرُ
Allāhumma bika amsaynā, wa bika aṣbaḥnā, wa bika naḥyā, wa bika namūtu wa ilaykal-maṣīr.
O Allah, by You we enter the evening and by You we enter the morning, by You we live and by You we die, and to You is the final return. (Hisn al-Muslim translation (sunnah.com), adapted to follow the Arabic word for word)

5.Sayyid al-istighfar, the best way of asking forgiveness
Authentic Prophetic duaاللَّهُمَّ أَنْتَ رَبِّي، لاَ إِلَهَ إِلاَّ أَنْتَ، خَلَقْتَنِي وَأَنَا عَبْدُكَ، وَأَنَا عَلَى عَهْدِكَ وَوَعْدِكَ مَا اسْتَطَعْتُ، أَعُوذُ بِكَ مِنْ شَرِّ مَا صَنَعْتُ، أَبُوءُ لَكَ بِنِعْمَتِكَ عَلَىَّ وَأَبُوءُ لَكَ بِذَنْبِي، فَاغْفِرْ لِي، فَإِنَّهُ لاَ يَغْفِرُ الذُّنُوبَ إِلاَّ أَنْتَ
Allāhumma anta Rabbī, lā ilāha illā ant, khalaqtanī wa anā `abduk, wa anā `alā `ahdika wa wa`dika mastaṭa`t, a`ūdhu bika min sharri mā ṣana`t, abū'u laka bini`matika `alayya, wa abū'u laka bidhanbī, faghfir lī, fa'innahu lā yaghfirudh-dhunūba illā ant.
O Allah, You are my Lord, there is none worthy of worship but You. You created me and I am Your slave. I keep Your covenant and my pledge to You as far as I am able. I seek refuge in You from the evil of what I have done. I admit to Your blessings upon me, and I admit to my sins, so forgive me, for none forgives sins but You. 6.O Allah, grant me health in my body
Authentic Prophetic duaاللَّهُمَّ عَافِنِي فِي بَدَنِي، اللَّهُمَّ عَافِنِي فِي سَمْعِي، اللَّهُمَّ عَافِنِي فِي بَصَرِي، لاَ إِلَهَ إِلاَّ أَنْتَ. اللَّهُمَّ إِنِّي أَعُوذُ بِكَ مِنَ الْكُفْرِ، وَالفَقْرِ، اللَّهُمَّ إِنِّي أَعُوذُ بِكَ مِنْ عَذَابِ القَبْرِ، لاَ إِلَهَ إِلاَّ أَنْتَ
Allāhumma `āfinī fī badanī, Allāhumma `āfinī fī sam`ī, Allāhumma `āfinī fī baṣarī, lā ilāha illā ant. Allāhumma innī a`ūdhu bika minal-kufri wal-faqr, Allāhumma innī a`ūdhu bika min `adhābil-qabr, lā ilāha illā ant.
O Allah, grant me health in my body. O Allah, grant me health in my hearing. O Allah, grant me health in my sight. There is none worthy of worship but You. O Allah, I seek refuge in You from disbelief and poverty, O Allah, I seek refuge in You from the punishment of the grave. There is none worthy of worship but You. 7.O Allah, I ask You for pardon and well-being
Authentic Prophetic duaاللَّهُمَّ إِنِّي أَسْأَلُكَ الْعَفْوَ وَالْعَافِيَةَ فِي الدُّنْيَا وَالآخِرَةِ، اللَّهُمَّ إِنِّي أَسْأَلُكَ الْعَفْوَ وَالْعَافِيَةَ: فِي دِينِي وَدُنْيَايَ وَأَهْلِي، وَمَالِي، اللَّهُمَّ اسْتُرْ عَوْرَاتِي، وَآمِنْ رَوْعَاتِي، اللَّهُمَّ احْفَظْنِي مِنْ بَينِ يَدَيَّ، وَمِنْ خَلْفِي، وَعَنْ يَمِينِي، وَعَنْ شِمَالِي، وَمِنْ فَوْقِي، وَأَعُوذُ بِعَظَمَتِكَ أَنْ أُغْتَالَ مِنْ تَحْتِي
Allāhumma innī as'alukal-`afwa wal-`āfiyata fid-dunyā wal-ākhirah. Allāhumma innī as'alukal-`afwa wal-`āfiyata fī dīnī wa dunyāya wa ahlī wa mālī. Allāhummastur `awrātī, wa āmin raw`ātī. Allāhummaḥfaẓnī min bayni yadayya, wa min khalfī, wa `an yamīnī, wa `an shimālī, wa min fawqī, wa a`ūdhu bi`aẓamatika an ughtāla min taḥtī.
O Allah, I ask You for pardon and well-being in this world and the next. O Allah, I ask You for pardon and well-being in my religion, my worldly affairs, my family and my wealth. O Allah, conceal my faults and calm my fears. O Allah, guard me from in front of me and behind me, from my right and my left, and from above me, and I seek refuge in Your greatness from being taken unawares from beneath me. Source: Sunan Abi Dawud 5074 · Sahih (al-Albani)Hisn al-Muslim no. 84 Ibn 'Umar said the Prophet ﷺ never left these words in the evening and the morning. The wording above combines Abu Dawud 5074 and Sunan Ibn Majah 3871, as Hisn al-Muslim does.
8.O Allah, Knower of the unseen and the seen
Authentic Prophetic duaاللَّهُمَّ عَالِمَ الغَيْبِ وَالشَّهَادَةِ فَاطِرَ السَّمَوَاتِ وَالْأَرْضِ، رَبَّ كُلِّ شَيْءٍ وَمَلِيكَهُ، أَشْهَدُ أَنْ لاَ إِلَهَ إِلاَّ أَنْتَ، أَعُوذُ بِكَ مِنْ شَرِّ نَفْسِي، وَمِنْ شَرِّ الشَّيْطانِ وَشَرَكِهِ، وَأَنْ أَقْتَرِفَ عَلَى نَفْسِي سُوءاً، أَوْ أَجُرَّهُ إِلَى مُسْلِمٍ
Allāhumma `ālimal-ghaybi wash-shahādah, fāṭiras-samāwāti wal-arḍ, Rabba kulli shay'in wa malīkah, ash-hadu an lā ilāha illā ant, a`ūdhu bika min sharri nafsī, wa min sharrish-shayṭāni wa sharakih, wa an aqtarifa `alā nafsī sū'an, aw ajurrahu ilā Muslim.
O Allah, Knower of the unseen and the seen, Creator of the heavens and the earth, Lord and Sovereign of everything, I bear witness that there is none worthy of worship but You. I seek refuge in You from the evil of my soul and from the evil of Satan and his snares, and from bringing evil upon myself or causing it to reach a Muslim. 9.In the name of Allah, with whose name nothing can harm
Authentic Prophetic duaبِسْمِ اللَّهِ الَّذِي لاَ يَضُرُّ مَعَ اسْمِهِ شَيْءٌ فِي الْأَرْضِ وَلاَ فِي السّمَاءِ وَهُوَ السَّمِيعُ الْعَلِيمُ
Bismillāhil-ladhī lā yaḍurru ma`as-mihi shay'un fil-arḍi wa lā fis-samā', wa huwas-Samī`ul-`Alīm.
In the name of Allah, with whose name nothing on earth or in the heavens can cause harm, and He is the All-Hearing, the All-Knowing. (Hisn al-Muslim translation (sunnah.com))

10.O Ever-Living, O Sustainer
Authentic Prophetic duaيَا حَيُّ يَا قَيُّومُ بِرَحْمَتِكَ أَسْتَغيثُ أَصْلِحْ لِي شَأْنِيَ كُلَّهُ وَلاَ تَكِلْنِي إِلَى نَفْسِي طَرْفَةَ عَيْنٍ
Yā Ḥayyu yā Qayyūmu biraḥmatika astaghīth, aṣliḥ lī sha'nī kullah, wa lā takilnī ilā nafsī ṭarfata `ayn.
O Ever-Living, O Sustainer of all, by Your mercy I seek help. Set right all my affairs, and do not leave me to myself even for the blink of an eye. (Hisn al-Muslim translation (sunnah.com), adapted to follow the Arabic)

11.Glory be to Allah and praise be to Him (100 times)

12.None has the right to be worshipped but Allah alone (10 times)
Authentic Prophetic duaلاَ إِلَهَ إِلاَّ اللَّهُ وَحْدَهُ لاَ شَرِيكَ لَهُ، لَهُ الْمُلْكُ وَلَهُ الْحَمْدُ، وَهُوَ عَلَى كُلِّ شَيْءٍ قَدِيرٌ
Lā ilāha illallāhu waḥdahu lā sharīka lah, lahul-mulku wa lahul-ḥamd, wa huwa `alā kulli shay'in qadīr.

Said ten times, it equals freeing four slaves from the descendants of Isma'il (Sahih Muslim 2693). The narration specifying ten times in the morning was graded hasan by Ibn Hajar.
13.I seek refuge in the perfect words of Allah
Authentic Prophetic duaأَعُوذُ بِكَلِمَاتِ اللَّهِ التَّامَّاتِ مِنْ شَرِّ مَا خَلَقَ
A`ūdhu bikalimātillāhit-tāmmāti min sharri mā khalaq.

Sahih Muslim (2709) also reports the Prophet ﷺ telling a man stung by a scorpion that had he said it in the evening, it would not have harmed him.
